What is Ultherapy?

Ultherapy or Ulthera is a non-invasive skin tightening procedure that utilises MicroFocused Ultrasound with Visualisation (MFU-V) to gently heat the different layers of skin to stimulate collagen production. Developed and manufactured in USA, Ultherapy is US FDA-cleared to lift and tighten skin on the brow, face, jawline and double chin

It is used to treat mild to moderate skin ageing concerns such as:

Ultherapy uses micro-focused ultrasound (MFU) energy to target various layers of the skin. The ultrasound energy is delivered at specific depths - 1.5mm, 3.0mm and 4.5mm, to effectively achieve visible tightening by stimulating collagen and elastin production.

Targeted ultrasound energy is used to heat different layers of the skin to trigger the body’s natural healing response. This, in turn, causes the body to regenerate collagen, which non-invasively lifts and tightens the skin. The levels of the skin that is targeted can be visualised using Ultherapy’s patented ultrasound technology, DeepSEE®.

  • Epidermis & Superficial dermis (1.5mm) — The outer layers of the dermis where Ultherapy improves skin texture and tightens the skin.
  • Deep dermis (3.0mm) — With focus on the dermis, Ultherapy can stimulate collagen and elastin production to improve skin firmness and elasticity. Fat can also be targeted, providing a more sculpted effect.
  • Superficial muscular aponeurotic system (4.5mm) or SMAS — the SMAS is a matrix in our skin rich in collagen and elastin fibres [1]. The high ultrasound energy heats up these targeted points called Thermal Coagulation Points up to 65˚ C and denatures existing collagen, causing these fibres to contract, while stimulating production of new collagen [2]. Over time, the skin is visibly lifted and tightened.

  • See — Ultherapy offers visualisation-aided ultrasound (MFU-V) using its patented DeepSEE® technology, allowing the doctor to target the specific layers within the skin. Being able to see into the layers of the skin also allows practitioners to avoid damaging bone and blood vessels, hence reducing chances of injury. 

  • Plan — In the Plan phase, your doctor will map out the collagen-rich tissues and come up with a treatment plan based on your skin’s composition and structure.
  • Treat — In the Treat phase, the doctor carries out the treatment and adjusts the depth and power according to the thickness of fat and skin to achieve an optimal outcome.

Ultherapy is suitable for individuals who are looking for a non invasive, non surgical face lift to reverse signs of facial ageing and address loose skin. It is ideal for individuals with mild to moderate signs of skin ageing who desire visible results in a single session.

Ultherapy can treat signs of ageing such as:

As Ultherapy is a personalised skin tightening treatment, a thorough consultation with your doctor is essential to assess your skin condition, address your concerns and come up with a tailored treatment plan.

The treatment starts with the application of a topical anaesthetic to ensure that you’re kept as comfortable as possible throughout the procedure.

The skin is then cleansed, and the areas of concern are mapped out. Your doctor will proceed to apply a cooling ultrasound gel and treatment is delivered via a specialised handheld transducer. Real-time skin layer visualisation is done using patented DeepSEE® technology, which allows your doctor to accurately deliver the ultrasound treatment without affecting other structures of the skin.

Ultherapy treatment takes 30-45 minutes, depending on the number of treatment lines delivered. Results are immediately visible and continue to improve with time, peaking at 2-3 months after treatment.

No, Ultherapy is not very painful, with most clients experiencing a slight pricking/tingling sensation that is tolerable with topical anaesthesia (numbing cream). After the treatment, patients may experience mild soreness for 3-5 days. There is the option of oral painkillers and oral sedation for individuals with a low pain tolerance.

From personal experience when I had my Full Face Ultherapy treatment in March 2024, Ultherapy does sting a little (especially at the jawline area), and I would rate the pain level as 3/10. You quickly get used to the sensation, which feels like tiny antbites.

There is no downtime after Ultherapy, clients can return to normal activity (eg office work, exercise) right away. You may experience mild swelling and a dull ache for 3-5 days.
Ultherapy is recommended as an annual treatment and should be done every 9-12 months. Younger individuals (age 25-35) with very mild facial sagging require 400-600 Ultherapy lines, while mature individuals (age 35 onwards) with moderate facial sagging require 600-800 Ultherapy lines to achieve a good outcome.
Results from Ultherapy can last up to one year or longer — this depends on lifestyle practices and your body’s individual characteristics [1].

Ultherapy is safe when performed by licensed and trained doctors, it is also different from other common skin lifting procedures as it is non-invasive and non-surgical. Ultherapy is the only FDA-approved non-invasive procedure for skin lifting and skin tightening since 2009, with over 100 publications detailing the efficacy and safety and over 3 million completed Ultherapy treatments worldwide. 

Furthermore, Ultherapy does not target melanin, meaning that it can be used safely in darker skin types [4]. However, Ultherapy does not resolve pigmentation, which requires lasers to treat.

Ultherapy is generally safe with adverse effects infrequent and temporary

Common adverse effects after Ultherapy

  • Bruising : due to injury of a blood vessel
  • Welts (swelling) : due to excessive heat in the skin, poor transducer contact

The above adverse effects are temporary and usually resolve within a week

Rare adverse effects after Ultherapy

  • Facial fat atrophy: individuals developing a “sunken” and “hollow” appearance after Ultherapy. Whether or not Ultherapy “burns fat” is a controversial topic, but doctors are starting to acknowledge that a handful of patients develop facial fat atrophy after Ultherapy.
  • Nerve injury: Numbness, tingling, shooting pain that can last for weeks to months. Again, a controversial topic with only a handful of case reports, probably under-reported in the literature. 

So the conclusion is that Ultherapy while generally very safe… can have rare but potentially serious consequences! Just like air travel or driving a car… you want to ensure your provider is highly experienced and well trained.

Ultherapy is contraindicated for patients with:

  • Open wounds on the face/neck
  • Metallic implants on the face/jaw
  • Signficant medical issues which may impair healing (eg cancer on chemotherapy)

Ultherapy is not recommended for patients with:

- advanced sagging of the face and neck and want a major result : these patients would probably benefit more from a surgical face lift

Ultherapy vs Monopolar RF (Oligio RF or Thermage FLX)

Ultherapy targets the deeper layer of skin for more face lifting effects, while monopolar RF such as Oligio RF or Thermage FLX targets the superficial layer of skin for more skin tightening effects. Oligio RF or Thermage FLX usesmonopolar RF, while Ultherapy utilises microfocused ultrasound waves. Monopolar RF generates bulk heating in the superficial skin layers at 1-2 mm depth, whereas focused ultrasound creates discrete thermal coagulation points at depths of 1.5 mm, 3 mm, and 4.5 mm.
